What Procedures Does Dr. Laker Perform?

The overwhelming majority of my practice focuses on management of both common abdominal procedures and complex procedures performed in a laparoscopic manner.

The following can be performed laparoscopically:

Adrenal Gland

The adrenal glands are mustard colored, small triangular or crescent shaped organs that are located on the upper pole of both kidneys. A layer of fatty tissue separates the adrenal glands from the kidneys. The adrenal gland is considered an endocrine organ in that it secretes substances into the blood stream that have a major impact on physiologic functions. During periods of stress (exercise, starvation, fear) the adrenal glands produce and release a number of hormones that optimize the body to best manage the stressors.

Appendectomy

The appendix is a slender tube that comes off the first portion of the colon (cecum). Despite being a well-known structure amongst physicians and lay persons alike its purpose is not well understood. In children and adolescents the appendix harbors a number of lymphocytes (immune cells) and may be important in immunologic development. Over time the number of lymphocytes appears to diminish and possibly its immunologic importance.

Bariatric Surgery

The term bariatric comes from the Greek word “baros” and loosely translates as “weight”. Bariatric surgery is the practice of surgery that aids in weight reduction. Over the last 75 years there have been various surgical methods that have been used to induce weight loss. A few of the initial procedures have been associated with severe long-term health issues and have been completely abandoned. Other methods failed to demonstrate sustainable weight loss or were likely to end with significant weight regain. The procedures that are used today are variations of prior procedures.

Biliary Disease

The gallbladder is a pear shaped hollow organ that is located under the right lobe of the liver. The gallbladder is connected to the liver by a network of tubes that are referred to as ducts. The primary function of the gallbladder is to store and concentrate bile. Bile is made in the liver and is secreted into the duct system. The bile travels from the liver and enters the small intestine within inches from the stomach. Bile enables the fats and oils we eat to be digested and made soluble in a water environment. This allows fats and oils as well as certain fat soluble vitamins to enter into the body and be used as a source fuel or to meet other metabolic needs. When there is a surplus of bile production by the liver the gallbladder becomes a storage depot. The “extra” bile can be stored and concentrated in the gallbladder. When a meal requires a larger volume of bile the gallbladder is triggered to contract and provide an additional amount of bile to aid in digestion.

Colon Surgery

The colon, also known as the large intestine, is a hollow organ of approximately 5 feet in length. Although much shorter in length than the small intestine, the term “large intestine” refers to its much larger diameter.

The colon is divided into anatomical segments based off blood supply and adjacent organs. The first portion of the colon is the cecum. The appendix projects off the cecum and within inches is the junction between the small intestine and colon. This is referred to as the ileocecal junction. The colon generally forms an inverted “U” as it ascends the right side of the abdomen (ascending colon), crosses the abdomen from right to left (transverse colon) and descends down the left abdomen(descending colon) to the “S” shaped sigmoid colon. The sigmoid colon empties into the rectum. Although the rectum is a continuation of the sigmoid colon there are a host of anatomic, mechanical and functional differences. Diseases of the colon and rectum are often managed in a different fashion.

Hernias of the abdominal wall

A hernia refers to tissue or an organ from one body cavity moving into another space where it should not normally be. In the majority of cases hernias are related to defects of the abdominal wall or diaphragm. The given medical “name” is based off of the location and nature of the hernia.

A hernia is not something that is removed. Hernias are repaired. A punctured bicycle tire can be repaired in a number of ways. The tire puncture could be glued primarily, a patch placed over the opening or perhaps below the opening. Regardless of technique, the problem area remains, it is not removed.

The small intestine is a long tubular organ that measures approximately 8 to 11 feet in length. The term “small intestine” refers to the narrow diameter of the organ relative to the colon or “large intestine”. The small intestine begins at the end of stomach and ends with its attachment to the colon. Anatomically the small intestine is divided into three areas: the duodenum, jejunum and ileum.

Stomach Surgery

The stomach is a dynamic organ that lies in the upper abdomen. In fact, more than 50% of the stomach is behind the lower aspect of the ribs and sternum. The stomach is responsible for accommodating ingested food and breaking the food-stuffs into smaller particles for digestion and absorption as the particles pass through the small intestine.
